Click on a form below, which opens in Adobe Acrobat, fill out the form, print, sign, and return to the Village Clerk's office at Village Hall, 3 W. Main St. (across from Walgreens) or email to clerk@villageofleroy.org. PLEASE NOTE: Forms won't be accepted unless signed.

How To Fill Out Form and Print:

When the form is open in Acrobat, click on the "Fill & Sign" feature on the left-hand tool bar under "Sign", fill out the form, then print from there or download/save to your computer and print.

OR

When the form is open in Acrobat, print a blank form, then fill out by hand.
